Programs & Strategies

La Clinica’s model integrates clinical and community health approaches to build a community of care. As a federally qualified health center, we provide accessible, holistic health services in a safe and supportive environment. Our community-based services increase language access and linkage to care, promote health, and engage our community in action towards health equity.

Clinical Services

La Clinica employs a team-based model of care. We believe that developing strong partnerships with our patients and their families is the best way to achieve and maintain health.

Mental Health & Substance use

At La Clínica, we are committed to meeting all physical and behavioral health needs of our patients. Our holistic approach to care sees mental health as a vital component to healthy living.

Community Health Action

Our Community Health Action (CHA) department identifies, educates, and engages members of the community through health promotion and wellness activities, women’s reproductive health and gender-based violence prevention, LGBTQ empowerment, HIV/AIDS testing, counseling and referral, and chronic disease management.

Health Equity

Our Health Equity Department is organized around 3 core elements (Strategic Partnership, Language Access, and Mapping Community Barriers to Health) and seeks to remove access barriers related to language, legal and social needs to improve health outcomes.

Strategies

We develop and employ innovative, culturally competent and linguistically appropriate strategies to break down barriers to health and healthcare access in our community.
